I just saw the movie Willard. This might have to top the Troma film Tromeo and Juliet as the most disturbing film I have ever seen. Featured: animal cruelty, human cruelty, very convincing unhinging of people, death, inspiring depression, etc. I don't think I can recommend it -- it is a very powerful film, but it is aimed at bringing out deep hurting, and it does. It's not at all campy, and it is an excellent film, but it inspires a very disturbed, freaked out mood.
